Comes in a negative mood.  Needs to be spoken directly to get him started on his morning work.

He is an extreme perfectionist.... cannot take criticism of any kind.....  He generally gets angry or frustrated and either throws his work away in the garbage or tears it up.

....when he is working on something challenging, he becomes nervous or anxious and begins to rock back and forth or side to side in his chair.

Often he is sitting out at P.E.

When Christopher is having a hard day he chooses to not socialize on the playground.  He will run around by himself and does not seem to want to play with others.

Comments from evaluation- March, 2008

Christopher’s mother reports that she was aware of his behavioral difficulties in preschool, kindergarten, and this year in first grade.

Interventions tried and brought to the CIT committee include behavior charts, power chair, removal from classroom, and visits with counselor.  Reportedly, Chrisopher has had difficulty recovering from episodes and may tear up or scribble on behavior charts.

Information from the classroom teacher indicates that Christopher has difficulty retaining instruction/information, difficulty following directions, extreme mood swings, and difficulty working with peers.  He also has difficulty with criticism and/or failure…… Instructional modifications and/or services that have been tried in response to Christopher’s difficulties include alternate materials, ability grouping, change in seat, behavior management, modified or shortened assignments, counseling, and school health services.

During testing, Christopher was, on occasion, observed shaking his head from side to side and clearing his throat.  He sometimes appeared inattentive, which seemed to interfere with his ability to follow oral instructions for certain subtests. 

He has some difficulty cooperating or complying with teacher requests and working cooperatively with others.  Reportedly, Christopher has particular difficulty adapting to new situations without getting upset, accepting responsibility for his actions, making and keeping friends at school, maintaining an even/usually happy disposition, responding appropriately to praise and correction, and resisting discouragement by difficulties or minor setbacks.

Behavior ratings by Christopher’s teacher produced significant externalizing concerns in the areas of hyperactivity, aggression, and conduct problems.  Christopher is reported as often being restless and overactive, and he has difficulty controlling his impulses...  Christopher has difficulty adapting to changing situations and it takes him longer to recover from difficult situations than most others his age...  A moderate concern was noted in Christopher’s ability to make decisions and get others to work together effectively.

4-24-08  (Teacher) reported that Christopher has made a lot of progress the past few weeks.  He is controlling himself more and making better choices.

4-24-08  Mother reported that Christopher is taking a vitamin regimen that may be attributed to his change in behavior, and they have noticed a change at home.

10-17-08  Christopher is doing very well in accepting classroom instruction.  In observations he has been attentive, focused and compliant with teacher directives given individually and to the whole group.

12-19-08  He is always a good model of self control and on task behaviorChristopher puts forth great effort in everything that he does…  Christopher is such a joy to have and brings so much personality to our class.

3-13-09  He continues to be a model student both academically and behaviorally. Christopher is always ready to participate in classroom discussions and can easily articulate his thoughts and feelings.  Christopher is a delight to have in my class each day.

5-29-09  Christopher has had a very successful 2nd grade year…  he is so mindful of others and has such a sweet personality.  He is such a wonderful student, he will be missed.

10-16-09  Christopher has been completely independent and has been redirecting himself during anxiety provoking situations…  he is a fantastic student academically.

12-18-09  He easily acquires new concepts…  His assignments are done on time and organization is not an issue.

12-18-09  Chris is an excellent all around student.  He is attentive and follows directions the first time given.  He works well with everyone in the class and is always eager to help anyone in need.  Chris always puts forth a good effort and has a positive attitude about school.  Chris is a joy to have in my class!

12-18-09  The general education teachers have not reported any concerns about his behavior and have nothing but positive things to say.

Comments from ARD dismissal- March 4th, 2010

Christopher has been doing great as always…  His teachers continue to report that he works really hard at completing his school work, is socializing with friends, and is easily re-directed when re-direction is needed. Christopher has not needed any additional support during anxiety provoking situations.

Christopher’s most recent report card indicates that he listens to and follows directions, respects the rights, property and opinions of others, works cooperatively with others, puts forth effort, is organized and prepared, produces legible work, completes work in a timely manner, and completes homework.

Christopher has mastered his behavioral goal and both objectives relating to self-regulation and coping strategies.  He has not needed behavior support services this year and has functioned very well independently. 

Christopher no longer has an educational need for special education services at this time.  The ARD committee agreed to dismissal at this time.

3-25-10  Chris is a pleasure to have in my class.  He is a steady worker and caring toward his friends.  He uses his time wisely and is always neat and organized.  Chris is supportive of others and has a great sense of fair play.

3-25-10  He is very organized and consistently does his best work.  Christopher always has a positive attitude about his work and his classmates.  He is a great asset to any class discussion and a joy to teach!

3-25-10  He quickly masters new topics and finds creative ways to problem solve.

6-02-10  He brought bright ideas and enthusiasm to our class discussions.  Chris is a role model to his peers and a great team leader!
